Admission for two to Dizzy’s Club Coca-Cola valid one time only for either May 19, May 26, June 2 or June 9, 2008 and excludes all holidays and special events.  Reservations subject to availability. There’s a $10 min at tables and $5 min at bar for all artist sets at Dizzy's Club Coca-Cola. Winner must acknowledge prize notification by contacting Jazz at Lincoln Center within three (3) days of date of notification.  If winner does not acknowledge notice or make arrangements during this time prize will be forfeited. Winner must contact Jazz at Lincoln Center at least three (3) days prior to desired performance date at Dizzy’s Club Coca-Cola to make reservations. Jazz CDs and merchandise must be claimed within one month of notification or prize will be forfeited.  Jazz at Lincoln Center/Dizzy's Club Coca-Cola is not responsible for unclaimed prizes.
